Living Well Consortium – The lAPT Long-term Conditions supports to treat anxiety, depression, and stress. The focus is on these conditions COPD, MUS, Cardiovascular disease. Anyone from age 16+.
We provide emotional well-being support using a stepped care model. Patients will receive support from a low-intensity psychological well-being practitioner, depending on needs, the patient can be stepped up to Hiph intensity practitioner.
This pathway is for clients who present with low mood, anxiety, and stress that impacts on their physical health and vice versa. Our practitioners will work from a psychological perspective and deploy various intervention tools, this may include exploring the client’s belief system of the disease, coping strategies, self-help management of symptoms and much more. Clients can expect to receive six to twelve sessions, which can be elevated to sixteen if required.
Waiting times on the LTC pathway are normally two to four weeks, however due to the unprecedented rise in mental health needs, clients may wait up to 12 weeks. During the wait clients can expect to recelve regular calls from wellbeing staff and offered self-help coping techniques.

In terms of NHS spending, at least £1. in every £8. is spent on long-term conditions linked to poor mental health. (Kings Fund).
